Key Responsibilities

• Conduct assessments and deliver evidence-based therapy for children aged 0–18

• Develop and review individualised therapy plans aligned with each child’s NDIS goals

• Provide therapy across clinic, school, home and community environments

• Maintain accurate and timely documentation, case notes and progress reports

• Collaborate with multidisciplinary team members to ensure holistic, family-centred care

• Provide advanced clinical reasoning and support team members during supervision

• Take students and supervise at least two mentees, providing high-quality guidance

• Identify training needs and deliver targeted in-services or workshops

• Act as a point of escalation for clinical or caseload concerns raised by junior staff

• Support diary management and caseload build-up for junior clinicians

• Build relationships with external stakeholders and represent the service where required

• AHPRA registration

• Current WWCC and NDIS Worker Screening Check

• Strong understanding of the NDIS framework and ability to navigate it confidently

• Excellent communication skills, written and verbal

We are seeking experienced community Registered Nurses nurses to provide healthcare in the home within a community in Regional Victoria.

The Position

• Community Healthcare provided within the Homes of residents in the community.

• Assess and document the anticipated needs of the patient.

• Make relevant referrals as necessary.

• Assess the occupational health and safety needs in the home and organize occupational therapy assessment to improve safety if necessary.

• In accordance with the prepared nursing care plan provide care that encourages the return to independent living where appropriate. 

• Liaise with the Clinical Case Manager and treating Doctor to maintain an exceptional standard of nursing care within the home.

• Observe, record and report to a Registered Nurse specific and general physical and mental conditions of patients, and symptoms and signs which may be indicative of change.

Requirements

• Three years post-grad acute experience.
