Job Description
We are seeking experienced community Registered Nurses nurses to provide healthcare in the home within a community in Regional Victoria.
The Position
- Community Healthcare provided within the Homes of residents in the community.

Assess and document the anticipated needs of the patient.

Make relevant referrals as necessary.

Assess the occupational health and safety needs in the home and organize occupational therapy assessment to improve safety if necessary.

In accordance with the prepared nursing care plan provide care that encourages the return to independent living where appropriate.

Liaise with the Clinical Case Manager and treating Doctor to maintain an exceptional standard of nursing care within the home.

Observe, record and report to a Registered Nurse specific and general physical and mental conditions of patients, and symptoms and signs which may be indicative of change.
Requirements
- Three years post-grad acute experience.
- Experience in healthcare in the home highly regarded.
- Drivers licence.
- AHPRA registration
- Australian Working Rights
