Community Based Clinical Care Manager – Registered Nurse | Sydney

Are you a Registered Nurse with a passion for community care? We’re seeking an experienced Clinical Care Manager (Community Based) to step into a permanent role with a leading not-for-profit aged care provider in Sydney. This is your chance to combine clinical leadership with hands-on community nursing while making a real impact.

About the Organisation:
Our client is a well-established not-for-profit with over 50 years’ experience providing aged care services across Metropolitan Sydney. They specialise in supporting older Australians from diverse cultural backgrounds, helping them navigate the aged care system and live active, independent lives at home.

About the Role:
This is a community-based position that blends clinical management with direct client care. You will:

  • Conduct clinical care assessments and reviews

  • Develop and oversee clinical care plans

  • Deliver high-quality nursing care to Home Care Package participants

  • Collaborate with support workers and multidisciplinary teams

  • Lead infection prevention and control (IPC) initiatives and provide staff training

  • Contribute to clinical governance and quality improvement systems

What We’re Looking For:

  • Bachelor of Nursing and current AHPRA registration

  • Minimum 2 years’ RN experience in Aged Care, Home Care, or NDIS (community experience preferred)

  • Strong clinical knowledge, including catheterisation and wound care

  • Proven ability to manage care planning and adapt to changing needs

  • Passion for supporting older Australians to live independently

  • Strong organisational and time management skills
